Download the Application Form and complete on your own computer or print the form and complete manually, ensuring that you provide as much information as possible. Please note that if you are currently living with a partner, the application will require both signatures.

Could you also enclose an accompanying letter outlining the summary of events that have led to your current situation?

You can post your signed application in an unstamped envelope, on which you have written in capital letters 'FREEPOST CAMERON', together with the following financial documentation (from both you and your partner if it is a joint application). Copies are acceptable.

  • Bank Statements for all current and saving accounts held, preferably the last three months;
  • Credit Card statement for each account you and hold;
  • Credit Agreements such as loans or Hire Purchase;
  • Mortgage Statement plus details of any arrears, repossession orders etc;
  • Further Information which relates to your current financial position;
  • Pro formas or estimates relating to your request for assistance, such as
    GP Returner training costs and fees, emergency house or car repairs.

The Cameron Fund seeks to monitor requests for assistance, in order to assess whether our applications fully reflect the GP population.  We would appreciate your completing the Ethnic Monitoring Form and returning it separately in an unstamped envelope marked 'FREEPOST CAMERON'.
